Remember to notify the office if this event is cancelled, rescheduled or changed in any way.  

When form is complete (which are also available in the School Office), give original to  Rod Webb or Rick Robertson, and give a copy to Becki Swartz.  

If Glenn Hayes has agreed to drive the bus, please place a copy in his box.
